P0128 Coolant Thermostat (Coolant Temperature Below Thermostat Regulating Temperature)
The engine isn't reaching its normal operating temperature in the time the computer expects — nearly always a thermostat stuck open.

The ECU knows roughly how long a healthy engine should take to warm up given ambient temperature and how far you've driven. P0128 means that deadline passed and coolant temperature never got there. It's one of the more clear-cut codes: the thermostat is the part whose job is exactly this, and it is the answer the large majority of the time.

How serious
Low for reliability, real for cost. An engine running below design temperature never leaves its warm-up fuelling, so economy drops noticeably, the heater underperforms, and long-term wear increases because oil stays thick and fuel dilutes it.

What causes P0128
Ordered by what to check first — how likely each cause is, weighed against what it costs to rule out. Working down the list in order is what stops you replacing an expensive part to fix a cheap problem.
Thermostat stuck open
The overwhelmingly common cause. A thermostat that fails open lets coolant circulate through the radiator constantly, so the engine can never build heat. Thermostats generally fail open rather than closed, which is fortunate — the closed failure overheats and destroys engines.
Wrong-temperature thermostat fitted
A cooler-rated thermostat, sometimes fitted deliberately in the belief it helps, will set this code because the ECU expects the factory temperature.
Faulty coolant temperature sensor
A sensor reading low makes a properly warm engine look cold. Compare its live reading against an infrared thermometer aimed at the thermostat housing — if they disagree, the sensor is your fault.
Low coolant level
Air pockets around the sensor produce erratic and low readings, and an underfilled system warms unevenly. Check level before anything else.
Cooling fan running constantly
A fan relay stuck closed, or a fan wired to run permanently, pulls heat out faster than the engine makes it — particularly noticeable in cold weather.
How to diagnose it
- Check the coolant level cold, and top up if low.
- Watch coolant temperature on live data through a full warm-up. A temperature that climbs and then falls back as you drive is the thermostat-stuck-open signature.
- Compare the scan tool's coolant temperature against an infrared thermometer on the thermostat housing to decide between a bad thermostat and a bad sensor.
- Feel the upper radiator hose during warm-up: it should stay cool until the thermostat opens. Getting hot straight away means the thermostat isn't closing.
- Check whether the cooling fan is running when it shouldn't be.
