P0401 Exhaust Gas Recirculation Flow Insufficient Detected
Not enough exhaust gas is being recirculated back into the intake — usually carbon has blocked the passages.
Emissions
The EGR system routes a metered amount of inert exhaust gas back into the intake to lower combustion temperature and cut nitrogen-oxide emissions. P0401 means the ECU commanded flow and measured less than expected. Because the whole system carries sooty exhaust through narrow passages, carbon buildup is by far the most common cause — and often the fix is cleaning rather than replacing.

Moderate. Higher combustion temperatures increase the risk of detonation and, over time, valve and piston damage. It's also an emissions failure.

What causes P0401
Ordered by what to check first — how likely each cause is, weighed against what it costs to rule out. Working down the list in order is what stops you replacing an expensive part to fix a cheap problem.
Carbon-clogged EGR passages
The dominant cause. The ports in the intake manifold and the tube to the EGR valve pack solid with carbon over years, especially on engines used for short trips. Cleaning the passages out is the actual repair — fitting a new valve to a blocked port accomplishes nothing, and this is where a lot of money gets wasted.
Stuck or failed EGR valve
The valve itself seizes with carbon or fails electrically so it never opens.
Blocked or cracked EGR tube
The tube between exhaust and valve packs with carbon or corrodes through.
Faulty EGR position sensor or DPFE sensor
The sensor that measures flow can fail and report insufficient flow when flow is fine. On many Ford applications the DPFE sensor is a known common failure and worth checking early.
Vacuum supply problem
On vacuum-operated systems, a cracked hose or failed solenoid means the valve never gets the signal to open.
How to diagnose it
- Remove the EGR valve and inspect both it and the port it bolts to. Carbon you can see is carbon that's causing this.
- Clean the valve and, importantly, the intake passages behind it with carb cleaner and a suitable pick or brush.
- Command the EGR valve open with a bidirectional scan tool at idle — a working system will stumble the idle noticeably.
- Check the EGR position or DPFE sensor reading against commanded position on live data.
- On vacuum-operated systems, check hoses and the control solenoid for leaks.
